Patient Information
New Patient Appointment
Our new patient appointments are about 90min with one of our two Doctors. Your first appointment at our office can either be a comprehensive dental exam and cleaning or a limited exam focused on investigating a primary concern you are having. In this case, we always hope to be able to perform treatment that day, but it will depend on the diagnosis. The following exam/dental cleaning appointments will most likely be scheduled with our hygienist, while one of our two dentists come in to do the exam. Our two doctors alternate days at our practice so either one can do your dental exam/treatment unless you have a preference, which would be fine, just request to be scheduled on their particular days. The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
Patient Forms
If you are a new patient to our office, please visit our patient forms page to complete our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Thank you and please contact our office if you have any questions at all.
Financial Protocol
Patients of Palacios Chen Family Dentistry pay the fee for dental services provided either upon checking out from their appointment, or following one of the alternative options listed below. We accept credit cards (Visa, MasterCard, Discover) cash or check.
We are out of network with dental insurance plans, but many of our patients have dental insurance and utilize their benefits in our practice. We happily take your insurance information, and work hard to maximize your benefits by submitting claims/records on your behalf. Patients pay the fee at the time of service, we submit the claim, and the insurance company sends the covered amount to the patient. If the insurance check does come to our office, we will forward the check on to our patient.
Payment protocol
· Fees are owed on the day of service or can be billed, to be due in 30 days
· Fees can also be paid for utilizing these three payment options:
o Pay in full same day of service by card, cash or check.
o Pay full balance spread out over 2-6 monthly interest free payments. This requires a credit card on file. The card will be charged during the first week of each month.
o Pay interest free monthly payments over the course of 7-24 months utilizing third party company Care Credit
Why are we out of Network?
There are a couple reasons that we as a practice remain out of network with dental insurance companies. First, being out of network prevents us from needing to rush through patient cleanings or treatment. It allows us to spend more quality time patients, form a relationship and a provide the highest quality personalized care we believe people deserve. It also helps to prevent insurance companies from entering the operatory impacting what a patient can or cannot do for their dental health. We believe dental treatment should depend solely on the goals and needs of the individual patient.
